Most final expense companies ask a series of health questions to determine eligibility. Below are seven of the most common. Your honest answers help determine which policy you may qualify for.

4Have you ever been diagnosed with Congestive Heart Failure, Cirrhosis of the liver, or End Stage Renal Disease?
5In the past 2 years, have you been diagnosed with or treated for any form of cancer (except basal or squamous cell skin cancer)?
6In the past 2 years, have you been diagnosed with, treated for, or advised to seek treatment for heart disease, stroke, or COPD?
7In the past 2 years, have you been hospitalized for any reason or advised to have surgery that has not yet been completed?
